YTS Parents,

I hope you guys are well. Please remember we have some deadlines to meet in terms of collecting paperwork, ordering uniforms and training t's.

Deadline for uniform order was Friday July 23rd. Ideally each family writes a check or gives you cash for the order they would like to place and the order is placed using one payment method preferably credit card. Hopefully you have provided all necessary info to your team manager so they can place the team order.


A US Club Medical Waiver. All families must complete this form even if they played YTS last year. Again your manager will collect these and turn them into the office at one time.

Training T-Shirt order form. As a reminder the YTS players are expected to wear CASL approved training attire to all YTS training sessions. This includes training shirt, black shorts and black socks. Again we’d like to have one order form per team. Once your manager has your respective size info the order can be placed.

Finally we need a copy of your son/daughter's birth certificate. Once you manager has received a copy they can turn all the info requested above into the CASL office and we will be good to go.

Clearly we need to work together to get all of the information collected. Please be responsive to all emails/requests.
